I understand that if I am playing a doubles game, then my doubles partner must still register. If they don’t register, my game will be cancelled.
I understand that if any player is aged 12 to 15 years, a parent or guardian must download and complete the waiver form. This form needs to be uploaded in order to complete my registration.
I understand that my doubles game will be cancelled if the doubles player named above does not register.
